Meaning of BMI Ranges

  • Less than 18.5: underweight
  • 18.5 to <25: normal weight
  • 25.0 to <30: overweight
  • 30.0 or higher: obese

Obesity is frequently subdivided into categories:

  • Class 1: BMI of 30 to < 35
  • Class 2: BMI of 35 to < 40
  • Class 3: BMI of 40 or higher, sometimes categorised as “severe” obesity.

There are exceptions and variations for certain ethnic backgrounds, age, and other factors where the healthy weight BMI range is generally:

  • lower for people of Asian background
  • higher for people of Polynesian background
  • higher for older people
  • higher for elite athletes with higher levels of lean body tissue
  • higher for pregnant women.

Because the BMI method doesn’t take fat or muscle into account,  it’s helpful to measure the waist as well.

How BMI Is Used

Overweight and obesity rates differ among citizens of  21 countries in the Organisation for European Co-operation and Development (OECD 2022).

Disease burden is an assesment of the number of years of healthy life lost from living in ill health or dying prematurely from diseases or injuries. A portion of this burden is due to modifiable risk factors.

The 2nd leading risk factor after tobacco use contributing to ill health and premature death, obesity is responsible for 8.4% of the total disease burden in Australia. In addition to the BMI method, there are other ways to assess body fat including the skinfold method, waist size, and waist-to-hip circumference.

Skinfold Method

Another way to estimate body fat, the skinfold method uses skinfold calipers to take skinfold measurements. This data can be helpful in determining health risks such as high blood pressure, heart disease, and other health concerns.

Waist circumference can help predict the risk of some chronic medical conditions.

A higher waist circumference can indicate internal fat deposits that can coat the heart, kidneys, liver, and pancreas, increasing the risk of chronic medical conditions like type 2 diabetes, heart disease, and cancer.

Men and postmenopausal women are more likely to develop excess fat in the waist area.

Among men, a waist circumference of 94 cm or more is related to an increased risk for chronic disease. Among men, a waist size of 102 cm poses a greatly increased risk. Among women, a waist circumference of 8o cm or more poses an increased risk, while a waist size of 88 cm or more poses a greatly increased risk.  Generally, the larger the waist, the higher the risk of developing chronic conditions exists.

While a helpful tool, waist size is just one indicator that may point to certain health conditions, and it is not used to singularly diagnose any condition or disease.

Waist-to-hip ratio (WTH) is a fast and easy way to assess fat distribution which can help determine a person’s overall risk for developing certain health conditions.

The WHR measurement involves using a tape measure to cheque the size of the waist and hips. WHR is found by dividing circumference of the waist by the circumference of the hips.

How to Measure WHR

Take waist tape measurements: Wrap a tape measure around the narrowest part of the stomach, near or just above the belly button.  

Take hip measurement by standing with feet directly beneath the hips and wrapping the measuring tape around the widest part of the hips and buttocks.  

To calculate WHR, divide waist size by hip size.

The World Health Organization has established guidelines when assessing waist circumference and waist-hip ratio. A healthy WHR is 0.9 or less in men and 0.85 or less for women.

Used to treat type-2 diabetes for 15 years,  the GLP-1 hormone prompts the body to produce more insulin, which works to control blood glucose levels.

When given in higher dosages, GLP-1 also interacts with the parts of the brain that suppress appetite and signal hunger satiation. When used in conjunction with diet and exercise, Semaglutide has not only shown to have promising results for the morbidly obese, obese, and overweight,  but it has been shown to reduce the risk of some types of cancer, diabetes, and cardiovascular disease.

As the medical community begins to accept obesity as a chronic metabolic disease instead of blaming the condition on a person’s lack of willpower or sedentary lifestyle, semaglutide has been shown to be effective in the age-old struggle with losing weight.  But semaglutide, though recognized as a scientific breakthrough by many,  is not a cure for obesity nor is it right for everyone.

Semaglutide for Weight Loss

As of the date of publication, Semaglutide has only been approved for weight loss under the brand name Wegovy. But as interest in semaglutide for weight loss continues to grow, and demand for Wegovy is outweighing supply, health care professionals have been prescribing other brands of semaglutide, such as Ozempic, for off-label use. Off-label is when a drug is used for a purpose other than the original reason it is TGA-approved.

But with so many people using semaglutide off-label for rapid weight loss, demand has become so high, that there is a shortage for people who need it as a diabetes medicine, the drug’s original purpose. 

Semaglutide Effectiveness for Weight Loss Among Non-Diabetics 

Though there have been other medications that work to suppress appetite, semaglutide performs on a whole new level.

In a study of obese adults, participants who only incorporated lifestyle changes lost about 2.4% of their weight, whereas 1/2 of the participants using semaglutide in conjunction with the exact same lifestyle changes  lost 15% of their body weight, with ⅓ losing as much as 20%.   • Anyone with a history of medullary thyroid carcinoma or with multiple endocrine neoplasia type 2;
  • gallbladder disease;

  • Or multiple endocrine neoplasia syndrome type 2 (MEN2)

  • Semaglutide injections are given as a shot under the skin on the upper stomach, thighs, or upper arm.
  • Alternate injection site each time you give yourself a shot.
  • Always check the liquid in the pen. It should be clear and colorless. Do not use it if it is cloudy, discolored, or has particles in it.
  • Use a new needle each time you inject your medicine.
  • Never share medicine pens with others under any circumstances. Sharing needles or pens can transmit infection.
  • Use this medicine on the same day each week with or without meals.

Dosing

The dose will be different for different patients. Follow your doctor’s directions and do not change unless your doctor tells you to do so.

Missed Dose

If you accidentally miss a dose, just take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double up doses.

  5. Repeat on right arm.

Legs/Lower Body

  1. Using gentle pressure, start massaging at the top of the left leg and work down toward the feet.
  2. Put one hand on the inside of the opposite thigh, near the groin, and place the other hand on the buttock.
  3. Gently stretch the skin, using sweeping motions, moving the hand on the inside of the thigh toward the outside of the thigh and up. 
  4. Using gentle pressure, move hands further down the the lower leg and repeat, stopping when you get to the knee.
  5. Place one hand on the shin and the other hand on the back of the calf and gently stretch the skin upward.
  6. Continue this motion, working down toward the ankle and top of the foot.
  7. Repeat on right leg.

  1. Use your thumb and index finger to guide fluid from the base of the toes toward the foot and the base of the fingers toward the palm.

Most common in teenage boys and older men, gynaecomastia can affect one or both breasts. Signs can vary from a small amount of extra fatty tissue around the nipples to more prominent breast development. Sometimes, male breasts can cause swelling breast pain. Male breast reduction surgery is just one way to treat the condition, though newer non-invasive treatments are surfacing every day.

Causes of Gynaecomastia

Hormonal imbalance – Hormone changes such as an imbalance in testosterone production and estrogen excess can produce gynecomastia. While all men produce some estrogen, they normally have much a higher testosterone ratio which stops breast growth.

An imbalance of hormones in the body can cause a man’s breasts to grow, though, the cause of this may be unknown.

Anabolic steroids – Bodybuilders that use anabolic steroids will often find that one of the side effects is the development of gynaecomastia. This is true gynaecomastia and a result of the growth of glandular tissue. Often nipples will point down, and in extreme cases, the pecs will look more like female breasts. Clearly, this is distressing when the goal is to achieve a muscular-looking chest with defined pecs. Discontinuing the use of steroids may help to prevent further growth of glandular tissue. To remove glandular tissue that has already developed surgical procedures for anabolic steroid-induced gynaecomastia is the only real option.

Obesity – Being overweight is one of more common causes of gynaecomastia. This is because being overweight can increase estrogen levels. Extremely overweight men are also more likely to have excess fat that can increase the size of the breast tissue. Newborn baby boys – Gynaecomastia can affect baby boys at birth, because estrogen from the mother passes through the placenta to the baby. This is usually temporary, however, and will disappear a few weeks after birth,

Puberty – Boys’ hormone levels vary during puberty. If testosterone levels drop, increased estrogen levels can cause breast tissue to grow. Teenage boys can have some degree of breast swelling from time to time. This is very common. In fact, more than 50% of all boys develop the condition during puberty but it usually clears up as they get older and hormone levels become more stable.

Ageing – As men age, testosterone levels tend to drop. Because older men have a tendency to have more body fat, this can cause more estrogen to be produced. These changes in hormone levels can lead to breast growth.

Types of Gynaecomastia

Real or Chronic Gynecomastia: Affecting more than half of adolescent males and 66% of male patients over the age of 50, real gynaecomastia is a condition that is commonly signalled by swollen breast gland tissue. Causes like a testicular tumor, advanced prostate cancer, androgen resistance, and an imbalance in testosterone and estrogen levels can lead to glandular tissue developing causing a man to develop gynecomastia. Painful gynecomastia tends to be associated with this form of the condition.

Pseudogynaecomastia: A term used to describe fatty tissue build-up along the chest area for male patients, patients with pseudogynecomastia may notice fat deposits that build up, which can appear similar to breast tissue on the chest area. Unlike severe gynaecomastia, which is caused by imbalances in oestrogen and testosterone in the body, pseudogynecomastia can be treated by eliminating the excess fat in the treatment area. The majority of male patients who have enlarged breasts tend to have a combination of fatty tissue and glandular tissue.

Mixed Gynaecomastia: A combination of excess fat and glandular tissue, mixed gynaecomastia may be treated with a combination of treatments.

Gynecomastia Diagnosed

A physical exam is the first step in determining the cause and type of male breasts before how the condition is treated. Enlargement of glandular tissue is the most challenging type of gynaecomastia to treat without surgery.

One or both breasts can be affected and surgical treatment is often the best solution for this severe form of the condition.

Medical Treatment for Enlarged Breast Tissue in Men

Hormone therapy may be used to treat gynecomastia. In some cases, gynecomastia surgery may be used to remove the extra tissue. SERMs) such as tamoxifen , raloxifene , and clomifene may be beneficial in the treatment of gynecomastia.

Clomiphene (Clomid) – A synthetic chemical taken orally, Clomiphene has been shown to partially reduce glandular tissue for approximately 50% of patients.

Tamoxifen (Nolvadex) – An oral medication, Tamoxifen is a type of selective estrogen receptor modulator (SERM).

Aromatase inhibitors – Aromatase inhibitors are a class of drugs used in the treatment of breast cancer in postmenopausal women and in men, and gynecomastia in men.

Anastrozole (Arimidex) – An aromatase inhibitor, Anastrozole can be effective for treating gynaecomastia in puberty.

Testosterone Replacement Therapy – TRT can be taken in various ways, including intramuscular injections, gel, and in stick form.

Non-Surgical Solutions for Pseudogynaecomastia

Pseudogynaecomastia is when hypertrophy of fat in the chest area gives the appearance of breast tissue or man boobs. This occurs in men that are overweight and can be reduced with a corrective diet and exercise to burn calories and decrease subcutaneous fat. When hormone levels are not balanced, some men find that avoiding certain foods could change estrogen levels and may also impact the growth of glandular tissue.

How Many Treatments Will I Need?

Several areas can be treated in as little as one 15-minute session, though additional treatments may be required to achieve the personalised results you desire.

TruSculpt Procedure

By delivering RF energy through the skin, the heat effectively destroys the unsightly fat without harming the skin. Once destroyed, the body flushes out the old fat cells through the lymphatic system.

The procedure, which takes approximately 15 to 60 minutes, depending on the number of areas being treated, feels similar to a hot stone massage. The majority of patients have not reported any pain or discomfort during  the procedure and no prescription pain medications, cooling gels, or compression garments are required. Mild redness, swelling, and tenderness can last from 10 to 12 hours, for which patients are advised to take mild OTC analgesics.

TruSculpt iD requires 2 sessions, 4-6 weeks apart.  Maximum results are normally visible after approximately 10-12 weeks after the last treatment, however some patients begin to see improvement after as little as 4 weeks. Whereas liposuction actually removes fat through a hollow tube and suction instrument, Trusculpt does not remove the fat, but rather causes fat cell death.  This disruption of cells allows the fat to leak and shrink and are gradually expelled naturally by the body.

What Causes Cellulite?

What is Liposuction? 

There are no two ways about it; liposuction is a surgical procedure involving anaesthesia, incisions, pain, and post-procedure recovery, which can last up to three months. Also known as suction-assisted lipectomy, the procedure involves removing small to moderate localised deposits of fat on targeted areas such as the abdomen, flanks, inner and outer thighs, and upper arms.

The liposuction procedure involves inserting an instrument called a cannula, a hollow tube that is inserted into the fatty areas by way of small incisions, and moving it around to loosen the excess fat. Similar to a vacuum cleaner, the fat is then suctioned out using a cannula connected to a vacuum or hand held syringe. Because of the movement inside the fat, the procedure causes significant trauma to the targeted area.

Because of the recovery time, pain, and cost, together with fears associated with surgery, the procedure has fallen out of favour now that less painful, non-invasive treatments.

Liposuction Recovery and Associated Post-Surgical Pain 

Just the thought of undergoing a surgical procedure to reduce body fat might be daunting for people thinking about liposuction surgery. The fear of bruising, discomfort, and downtime surrounding the procedure has also scared more than a few patients away.

One of the most significant roadblocks to liposuction for many people considering liposuction surgical fat reduction is the significant pain and discomfort most patients experience after the procedure.  Depending on the areas treated, patients can experience significant  pain when sitting, standing, walking, or getting out of bed for seven to ten days.

Liposuction Recovery Process

After the liposuction surgery is complete, patients are dressed in compression garments to reduce swelling. Some bruising, redness, tenderness, soreness, itchiness, and discomfort can be expected. Depending on the individual’s pain tolerance, there are some patients that do well with over-the-counter pain medications like Acetaminophen or Ibuprofen after liposuction, while many require prescription pain medications.

Though the compression garments help to keep swelling to a minimum, it can take several months for the swelling to subside completely. The swelling will reduce significantly with each passing week.

First Day After Liposuction Surgery

  • Expect to feel sore for the first 24 hours. Pain medication may be utilized at this time to ease discomfort. 
  • It is normal for bloody fluid to drain on clothes and sheets for the first 24 hours. Don’t worry! The fluid is put into the area before liposuction to make fat removal safer. 
  • Following the surgeon’s post-care instructions is imperative. Rest as much as possible during the first day.  Compression garments can be a little uncomfortable at this time and take some getting used to. 
  • Avoid unnecessary movement. Ask family and friends to assist you at this time.
  • Mild bruising, swelling, and some bloody fluid coming out of the incision is normal after surgery. These symptoms should gradually subside within 2–6 weeks.  

The First 72 Hours After Liposuction Surgery

  • It is common for bloody fluid to drain on clothes and sheets for the first 72 hours after surgery. Don’t worry! The fluid is put into the area before liposuction to make fat removal safer. 
  • Your compression garment should be worn at all times except during showering. Compression garments reduce swelling and speed up your recovery. Your plastic surgeon will let you know when you can stop wearing the garment.
  • It is usually advised that you move every two hours to prevent blood clots. This also helps keep fluids moving and reduce swelling.
  • Continue avoiding exercising during the first 72 hours.

The First Month After Liposuction Surgery

  • Though mild discomfort is normal for the first month, the first week is usually the most challenging period for patients and will subside as the month moves along. 
  • Some residual swelling is still expected after the first month, but at this point, you will start seeing some of the initial results.
  • If you work outside the home, you might be able to return to work and start light exercise after the first month.
  • Remember to follow your provider’s instructions to the letter. 

90 Days After Liposuction Surgery

  • ​​Three months after liposuction is normally when patients notice the treated areas beginning to appear tighter and smoother. 
  • Scars from the incisions will have faded at this point and should be barely noticeable and easy to hide.
  • By day 90, you should feel like you’re finally getting back to normal and results will be more apparent.

Can I Shower After Liposuction Surgery?

Even though liposuction surgery doesn’t  involve large incisions, it is important to know how to care for your body after the procedure. This includes bathing. Though some  surgeons will allow you to shower 24 hours after surgery, many may want you to keep your initial compression garments on for 2–3 days, which means you won’t be able to shower until after that period. When you do finally shower, run soapy water over the areas. Do not scrub or use a washcloth, loofa, or other bathing tool. Gently pat the area dry with a clean towel. Avoid  non-running water such as baths, hot tubs, swimming pools, lakes, rivers, and oceans for the first 2 weeks.

Will Fat Cells Return After Liposuction Surgery?

Because the body will not produce new fat cells as long as the patient maintains a stable body weight after the procedure, most patients do not experience a re-growth in fat cells in the treated area after liposuction. When Will I Notice Results After Liposuction Surgery?
